HOW TO SUBMIT FOR SGA 2026 

1. Follow these steps 

• Complete Part A Audit Questionnaire (Google Form)
• Append Part A supporting documents to Google Form submission
• Download this Part B and complete it 
• You may include initiatives or projects that began from September 2025 to date in your submission.
• Email the completed Part B submission form to sga@sec.org.sg

2. Please name all supporting documents according to the following:
